Please call the clinic and speak with one of the receptionists. She will need about 15 minutes to collect some initial information. She will then review the information with the appropriate clinician and will schedule an appointment.
Please come about 30 minutes prior to you scheduled appointment to complete initial paperwork. You may also print and complete the documents above, and arrive only 15 minutes before your scheduled appointment.
We accept most major commercial insurances. We do not accept T19/T18 insurance. We will contact your insurance prior to your appointment, but we highly recommend that you contact your insurance company directly as well. When you call request a mental health benefits summary including:
  • In network benefits/Out of network benefits
  • Calendar Benefit year
  • Percentage of coverage
  • Deductible, and if it has been met
  • Copay
  • Coinsurance
  • Maximum of benefits
  • Authorization or pre-cert requirement information
  • If obtained, authorization number, effective dates and number of sessions
  • Out of pocket maximum
  • Number of visits per year maximum

If your supply of medication is not sufficient to last until your next appointment with your prescriber, please plan ahead and call the clinic on Monday, Tuesday, Wednesday or Thursday morning with your pharmacy, and pharmacy phone number readily available.
